诊断助手 tool call 控制（ADR-197/155/203，纯 tool call UI）：LLM 意图解析 → 待确认队列 → 批处理执行 → tool 消息回填 → 继续 stream。parseActionFromLLM 三级容错提取 JSON；破坏性操作（action.destructive）前置 showConfirm；undo 委托 scene:undo 而非本地回滚。
